Job summary

Mersey and West Lancashire Teaching Hospitals NHS Trust is seeking an experienced Advanced Occupational Therapist (Band 7) to join the Frailty Therapy Intervention Team. The role focuses on complex frailty assessments, rehabilitation and discharge planning to promote independence and safe community return.

You will lead a senior multidisciplinary team, supervise junior staff, and contribute to service development across trust sites. A proactive, patient‑centred approach is essential.

Qualifications

• HCPC registration as an Occupational Therapist.
• Post‑registration experience in frailty management.
• Strong clinical reasoning and decision making.

Responsibilities

• Complete comprehensive OT assessments for complex frailty patients.
• Develop person-centred rehabilitation and discharge plans to promote independence and avoid hospital stays.
• Work with consultants, nursing, A&C Practitioners, physiotherapists, social care and community services for coordinated care.
• Provide specialist advice to the MDT.
• Supervise junior OTs, therapy assistants and students.
• Lead service improvement initiatives and contribute to audits and QI projects.
• Support evidence‑based practice and help shape frailty services direction.
