Job summary

Mersey and West Lancashire Teaching Hospitals NHS Trust is seeking an Advanced Occupational Therapist (Band 7) to lead in the Frailty Therapy Intervention Team. The post focuses on assessment, rehabilitation, discharge planning and admission avoidance for older adults living with frailty.

You will work across the frailty pathway in a senior MDT role, supervising staff and contributing to service development within a supportive team environment.

Qualifications

• Must have HCPC registration and an occupational therapy degree.
• Post-registration experience in frailty management is desirable.
• Clinical Educators Certificate or equivalent/experience is preferred.
• MSc in relevant health studies or equivalent qualification/experience advantageous.

Responsibilities

• Complete comprehensive occupational therapy assessments for patients with complex frailty needs.
• Develop person-centred rehabilitation and discharge plans to promote independence.
• Collaborate with consultants, nursing, ACPs, physiotherapists, social care and community services for coordinated care.
• Provide specialist advice to colleagues across the MDT.
• Supervise junior Occupational Therapists, Therapy Assistants and students.
• Lead service improvement initiatives and contribute to audits, QI projects and service evaluation.
• Support evidence-based practice and shape future frailty services within the Trust.
