You will provide rapid, holistic assessment and person-centred care for older adults living with frailty, supporting timely treatment and safe discharge or transfer.

Qualificações

• Registered nurse with NMC registration and current practicing licence.
• Experience in acute care settings is desirable.
• Ability to work in a fast-paced Frailty SDEC environment.

Responsabilidades

• The assessment, planning, implementation, and evaluation of nursing care
• Supporting shift coordination to ensure safe, efficient, and effective care delivery
• Collaborating with multidisciplinary colleagues including geriatricians, GPs, physiotherapists, and pharmacists
• Acting as a key link between primary, secondary and social care to reduce unnecessary admissions and support safe discharge planning
