You will work within a highly skilled multidisciplinary service providing rapid, holistic assessment and person-centred care for older adults living with frailty, supporting timely treatment and safe discharge or transfer.

This is an excellent opportunity to develop specialist expertise in acute frailty, rapid assessment, clinical decision-making, medication management and complex discharge planning. As a key member of theFrailty SDEC team, you’ll provide high-quality nursing care to older adults presenting with frailty syndromes. You’ll work closely with Emergency Department colleagues to proactively identify and manage patients, contributing to comprehensive geriatric assessments and the creation of supportive management plans.

You will be responsible for:

  • Supporting shift coordination to ensure safe, efficient, and effective care delivery

  • Collaborating with multidisciplinary colleagues including geriatricians, GPs, physiotherapists, and pharmacists

  • Acting as a key link between primary, secondary and social care to reduce unnecessary admissions and support safe discharge planning

You will help promote admission avoidance, reduce hospital length of stay, and support early intervention for individuals living with frailty.
