Job summary

Tameside and Glossop Integrated Care NHS Foundation Trust is seeking a Clinical Fellow to join the Frailty team, supporting older adults in the Emergency Department and Frailty SDEC service.

The post holder will work with Specialty Doctors and Consultants in a multidisciplinary team, delivering CGA and facilitating safe discharge, with supervision and teaching duties for junior doctors.

Responsibilities

• Participate in Consultant-led ward rounds within the Frailty Decisions Unit.
• Identify patients in the Emergency Department suitable for assessment and management within the Frailty SDEC service.
• Undertake Comprehensive Geriatric Assessments (CGA) as part of a multidisciplinary team under supervision.
• Deliver high-quality medical care in line with agreed clinical management plans.
• Facilitate safe and timely discharge with accurate documentation and GP communication.
• Ensure discharge plans are coordinated with community health and social care services.
• Support training and supervision of Foundation Year doctors and participate in Grand Rounds teaching.
• Participate in audit, quality improvement and governance activities as agreed with Educational Supervisor.
• Maintain compliance with data protection and information governance standards.
• Adhere to Trust policies, procedures and clinical guidelines.
• Undertake additional duties as required by Consultants or Specialty Doctors.
