South Tees Hospitals NHS Foundation Trust is seeking a Clinical Care Coordinator Band 7 to lead the Integrated Discharge Team based at North Tees. You will work with the Patient Flow Team, ward staff and local partners to ensure excellent care transitions and service delivery across sites.

The role requires strong leadership, communication and collaboration across professional boundaries, with responsibility for staff management, training, and programme development within the Transfer of Care

• Proven leadership and communication skills.
• Experience managing clinical teams is desirable.
• Familiar with NHS policies and governance.
