Job summary

Sirona care & health CIC in Weston-super-More is seeking a Senior Occupational Therapist for the Integrated Network Teams. The role focuses on home-based rehabilitation, leading a caseload and supervising junior staff and students in a six-month maternity cover or permanent capacity.

HCPC registration and evidence of leadership are essential. The post involves developing care plans, coordinating services, and contributing to ongoing professional development within the NHS framework.

Qualifications

• Evidence of leading a team with senior support when required.
• Evidence of supervising staff and students of own profession.
• Evidence of continuing professional development and commitment to lifelong learning.

Responsibilities

• Carry out initial holistic assessment, and implement and evaluate care programmes for acutely and chronically unwell patients, often with complex needs.
• Use clinical reasoning to assess, plan, implement and evaluate patient-centred interventions based on evidence.
• Manage own caseload and provide proactive service management.
• Allocate work and coordinate the service and team efficiently.
• Build communicative working relationships to prevent avoidable admissions and enable timely discharges.
• Negotiate achievable goals with service users, carers and family within the overall care plan.
• Make appropriate referrals to other services based on assessment.
• Assess and arrange equipment within scope of practice, reporting any issues.
• Develop other members of the INT, including peers, junior staff and students.
• Keep accurate, informative and legal records.
• Take part in training, which may include helping to deliver it.
• Identify risks and act to minimise risk to self, colleagues and service users.
