Job summary

Sirona care & health is seeking Band 5 Occupational Therapists to join the Integrated Neighbourhood Team in Weston. You will deliver community-based rehabilitation, working with a multidisciplinary team to maximise independence and quality of life for service-users in their homes.

Qualifications

• Holds UK HCPC registration and a BSc in Occupational Therapy.
• Demonstrates ongoing professional development and lifelong learning.
• Understands and applies evidence-based practice in community rehab.

Responsibilities

• Conduct holistic assessments and develop care plans for acutely and chronically unwell patients.
• Provide patient-centred intervention using evidence-based approaches.
• Collaborate with service users, carers and MDT to set realistic goals.
• Arrange referrals and work within a multi-disciplinary team to deliver integrated care.
• Supervise peers and students, and contribute to training.
• Maintain accurate records and participate in training activities.
