Sirona care & health in Weston is seeking Band 5 Occupational Therapists to join our Integrated Neighbourhood Team. You will deliver patient-centred, home-based rehabilitation and support independent living in the community.

You will work within a friendly MDT, provide holistic assessments, plan and evaluate interventions, and help with equipment provision and training. NHS benefits and development opportunities are offered.

Formación

• Qualified Occupational Therapist with HCPC registration.
• Experience in community-based therapy is desirable.
• Excellent communication and collaboration skills.

Responsabilidades

• Conduct initial holistic assessments, implement and evaluate programmes of care for acutely and chronically unwell patients, often frail elderly.
• Apply clinical reasoning to plan, implement and evaluate patient-centred interventions.
• Collaborate with service users, carers and families to identify achievable goals within the care plan.
• Facilitate referrals to other services as needed.
• Work in a multi-disciplinary team to share skills and knowledge with robust competency framework.
• Deliver occupational therapy in functional and standardised assessments as an autonomous practitioner.
• Assess and arrange equipment, ensuring safe and proper fit; report issues.
• Supervise team members, peers, students and unregistered staff in OT competencies.
• Maintain accurate, informative and compliant records.
• Participate in training and contribute to service development.
