Job summary

Sirona care & health CIC is seeking a Band 5 Occupational Therapist to join our Integrated Neighbourhood Team in Weston-super-Mare. You will deliver holistic, patient-centred community therapy, support hospital discharges, and work within a multidisciplinary team to maximise independence in the home setting.

Qualifications

• Essential: BSc or equivalent in Occupational Therapy and HCPC registration.
• Essential: Evidence of continuing professional development and lifelong learning.

Responsibilities

• Conduct initial holistic assessment and develop care plans for acutely and chronically unwell patients.
• Use clinical reasoning to plan, implement and evaluate patient-centred interventions with evidence-based practice.
• Collaborate with service users, carers and family to set achievable goals within the care plan.
• Facilitate appropriate referrals to other services.
• Work in a multi-disciplinary team and share knowledge within a robust competency framework.
• Use occupational therapy skills in functional and standardised assessments as an autonomous practitioner.
• Assess and arrange equipment provision, ensure proper functioning and safe fitting.
• Supervise peers, students and unregistered staff in OT competencies.
• Maintain accurate, legal records and participate in training.
