Job summary

Gateshead Health NHS Foundation Trust is seeking a District Nurse to join a dynamic locality team covering the EAST of Gateshead. The role supports service users within the Sister Winifred Laver Promoting Independence Centre and collaborates with local authority, GP surgery, and multidisciplinary teams to deliver high-quality care.

The post requires NMC registration, a degree in Adult Nursing, and a current UK driving license.

Qualifications

NMC registration is mandatory.
Degree in Adult Nursing is required.
Non medical prescribing qualification is required.

Responsibilities

To assess the health and health related needs of patients, clients, their families and other carers and identify and initiate appropriate steps for effective care for the individuals and groups.
To assess and treat specific diseases in accordance with agreed medical protocols.
To assess, plan, provide and evaluate specialist clinical nursing care to meet care needs of individual patients and carers in their own homes.
To oversee the provision of individual patient care by personal participation in order to maintain skills as an expert practitioner and provide an effective role model for the nursing team and others in the multi-disciplinary team.
To analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.
To prescribe appropriate items from the Prescribing Formulary when required.
To work with Trust policies and procedures that ensure patient safety at all times
To manage patients identified as at risk so that they receive appropriate interventions to be able to improve their health and wellbeing and achieve their maximum level of independence
To manage patients that have complex needs and potentially numerous co-morbidities including frailty
