We are looking for an experienced and motivated Senior Nurse to join our friendly and supportive Community Nursing Team.
This is an exciting opportunity for a registered nurse with excellent clinical and leadership skills to provide high-quality care to patients within their own homes and community settings. You will be responsible for managing a caseload of patients with complex health needs, carrying out holistic assessments, planning and evaluating care, and working collaboratively with patients, families and partner agencies.
As a Senior Community Nurse, you will provide clinical leadership and support to the wider nursing team, helping to maintain high standards of patient care, quality and safety. You will play a key role in preventing unnecessary hospital admissions, supporting timely discharge from hospital and enabling patients to remain independent within their own homes whenever possible.
The successful candidate will be an effective communicator, able to work autonomously and as part of a multidisciplinary team. You will be committed to delivering compassionate, person-centred care and supporting continuous service improvement.
In return, we offer a supportive working environment, regular supervision, professional development opportunities and the chance to make a real difference to the lives of patients within the local community.
A full UK driving licence and access to a vehicle for work purposes are essential.
