Job summary

Procare Community Services is seeking a District Nurse to join our clinical community nursing team. You will visit patients in their homes and care homes, manage caseloads, and provide holistic nursing assessments and care.

The role requires a District Nursing Qualification and NMC registration, with driving to support home visits. A full UK driving license is preferred; we offer training and a supportive team environment.

Qualifications

• Registered Level 1 Nurse with current NMC registration.
• Experience in community nursing is desirable.
• Valid UK driving license is required or accommodation can be made.

Responsibilities

• Maintain strong relationships and communicate effectively with GPs, practice nurses, Social Services, and hospital colleagues to ensure high-quality care.
• Attend Gold Standard Framework and MDT meetings.
• Coordinate meetings with GP and Practice Nurse colleagues and manage caseloads for home-visit patients.
