Job summary

Gateshead Health is seeking an experienced Frailty Nurse Practitioner to join the Frailty Service, working across the Acute Frailty Team and Frailty Virtual Ward on a rotational basis. You will deliver specialist, person-centred care to people living with frailty in both hospital and community settings.

The role focuses on advanced clinical assessment, clinical decision-making, and collaborative care planning with the multidisciplinary team to support timely interventions and safe transitions

Qualifications

• Advanced clinical assessment and decision-making skills.
• Experience in frailty care and managing complex caseloads.
• Non-medical prescribing within scope of practice.

Responsibilities

• Work autonomously with advanced clinical knowledge to provide high-quality, person-centred care.
• Hold clinical responsibility for a defined caseload of patients with frailty and complex needs.
• Undertake comprehensive assessment, care planning, intervention and evaluation.
• Provide specialist clinical advice to patients, carers and the MDT on assessment, care planning and management.
• Develop, deliver and oversee complex case management across acute, community and Frailty Virtual Ward settings.
• Promote continuity and coordination of care across acute, community, primary care and social care services.
• Support patient choice and independence, delivering care closer to home where possible.
• Contribute to development, delivery and evaluation of frailty services.
• Maintain a culture of professional learning and evidence-based practice.
