Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a District Nurse to operate within the St Neots/A1 Community Nursing Team. You will provide autonomous home-based care, manage a complex caseload and work with GPs and a multidisciplinary team to prevent unnecessary hospital admissions.

The post requires a District Nurse Specialist Qualification or leadership experience, CPD evidence, and a plan for postgraduate community qualification.

Qualifications

• Requires District Nurse Specialist Qualification or leadership experience in community nursing.
• Evidence of CPD with plan for postgraduate community qualification.
• Must hold a full UK driving licence and have access to a vehicle.
• Able to travel independently around the county to meet service needs.

Responsibilities

• Work as autonomous practitioner with the integrated multi-professional team.
• Manage own caseload and assess complex patients.
• Plan, implement and evaluate treatment within professional guidelines.
• Keep patient at centre of care with patient and carer participation.
• Provide support and education to peers, new staff, and students.
• Supervise junior colleagues and delegate caseload accordingly.
• Deliver high standards of nursing care in patients' homes, including lone working.

Job description

An exciting opportunity has arisen for a District Nurse to work within our St Neots/A1 Community Nursing Team. You will be working as part of a team which includes Community Nurses (RN), Advanced Clinical Nurse Practitioner and Integrated Care Workers. You will link closely with colleagues outside of CPFT including GPs and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission. The successful candidate will need to have a District Nurse Specialist Qualification, or have worked in a leadership role within a community nursing team or similar role - with community related continual professional development evidenced with completion or plan for completion of post graduate community related qualification. The post holder will be supported and be committed to further development within role either with modular post graduate development or completion of the Specialist Practitioner DN Qualification., Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence
