Job summary

Gateshead Health NHS Foundation Trust is recruiting for a Community District Nurse/Clinical Nurse Practitioner to join the East Locality Team in Gateshead. The role focuses on delivering high-quality care in patients’ homes and within the community, coordinating with local partners to promote independence and safe practice.

You will be NMC-registered with an adult nursing degree, hold a valid UK driving licence and be prepared to take 24-hour responsibility for a caseload and lead a small team

Qualifications

• NMC registration and Degree in Adult Nursing are essential.
• Non medical prescribing is desirable.
• Experience working in a community setting is essential.

Responsibilities

• Deliver high standards of clinical practice ensuring safe, quality care.
• Identify and assess core needs of patients and carers, delivering identified packages of care.
• Assume 24-hour responsibility for a designated caseload.
• Lead and develop staff and the service provided.
• Work effectively in a multi-disciplinary/agency team.
• Delegate work according to skill mix while maintaining safety.
