Job summary

• Manage a caseload of patients with complex long-term conditions at high risk of hospital admission.
• Deliver intensive case management to support admission avoidance and timely discharge.
• Coordinate care with GPs, hospital teams, social care and voluntary organisations.
• Lead multidisciplinary team meetings and model best practice.
• Identify high-intensity service users and develop proactive care plans.
• Use advanced clinical skills, including prescribing where appropriate.
• Provide supervision, support and education to clinical staff and learners.
• Contribute to service improvement, quality assurance and transformation.
• Support use of assistive technology, health coaching and patient education.
• Participate in integrated care pathways to ensure timely right-care delivery.
• Deliver patient-centred care to promote independence and self-management.
