Job summary

Black Country Healthcare NHS Foundation Trust is seeking an experienced Band 6 community nurse to join our multidisciplinary team in delivering high-quality care to adults with learning disabilities in the community.

The post is full-time, with opportunities for clinical supervision, professional development, and working with social care partners and other agencies to support individuals in living well in the community.

Responsibilities

• Provide specialist health support to individuals, families and carers.
• Work as part of the multi-disciplinary team.
• Complete specialist health assessments as clinically indicated.
• Participate in referral meetings, team meetings and wider MDT meetings.
• Contribute to team and service development.
• Carry an individual caseload.
• Keep up-to-date with research and development in learning disability nursing practice.
• Attend training relevant to the role.
• Maintain NMC registration and stay current with research in learning disability nursing.
• Communicate with social workers, care providers and other agencies and services.
• Supervise and assess student nurses on placement with the team.
