Job summary

Midlands Partnership University NHS Foundation Trust’s Children’s Community Learning Disability team is expanding and seeks an experienced registered Learning Disability or Mental Health Nurse to support children and young people with learning disabilities and their families. You will work alongside a skilled MDT including psychologists, OT, SLT and psychiatrists to deliver person-centred care.

Qualifications

• Registered nurse with experience in learning disability and mental health.
• Experience working with children and young people.
• Ability to manage a complex caseload and develop care plans.
• Commitment to person-centred care and multidisciplinary collaboration.

Responsibilities

• Assess mental health and medical risk factors in children and young people.
• Conduct physical health monitoring and identify actions.
• Deliver interventions in line with care plans and pathways.
• Complete and contribute risk assessments and management plans.
• Maintain an effective reporting system on patient conditions.
• Utilise outcome measures to review intervention effectiveness.
• Work within the NMC Code of Practice and professional guidelines.
