Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Fareham is seeking a Band 6 Senior Community Nurse to provide strong holistic leadership to the community team and support the Community Matron. The role focuses on admission avoidance, early supported discharge, promoting independence, and coordinating care with GP practices, social care and voluntary sector.

Qualifications

• BSc Nursing or RN registered with the NMC and current UK registration.
• Post-registration qualification or a university degree is required.
• Teaching and assessing qualification ENB 998 or equivalent/mentorship qualification (PETALS) preferred.
• Management/leadership qualification or development programme encouraged.

Responsibilities

• Provide strong holistic leadership to the community team and support the Community Matron.
• Lead and facilitate patient-centred, co-ordinated care pathways across primary and secondary care.
• Identify patient needs and liaise with services to enable safe, early intervention.
• Promote flexible working and seven-day service delivery in line with Trust standards.
