The Coventry and Warwickshire Partnership NHS Trust is seeking an Intensive Support Team Nurse (Band 6) to support adults with learning disabilities across Warwickshire. This part-time secondment (20 months) offers 30 hours per week, with base choices at The Loft, Nuneaton or Whitnash Lodge, Leamington, while covering across the county.

You will join a MDT-focused community team delivering intensive assessment, planning and interventions to prevent hospital admissions and support discharge.

Qualifications

  • Nursing Degree in Learning Disability or Mental Health Nursing is essential.
  • Registered with the NMC is essential.
  • RNLD or RNMH with substantial post registration experience is essential.
  • Experience in multi-agency teams with multi-disciplinary colleagues is desirable.
  • Must have a valid full UK Driving licence.

  • Empower and promote individuals with learning disabilities to live meaningful lives.
  • Adopt Positive Behaviour Support approaches to reduce hospital time and prevent readmissions.

Adult Learning Disability Nurse, Band 6

We are excited to be able to offer a full-time band 6 nursing role within the Intensive Support Team.

We are a community-based team that provides intensive assessment and interventions to individuals who are at risk of admission to hospital with the aim of avoiding unnecessary admissions, as well as supporting individuals through discharge transition out of hospital. We work in close partnership with individuals, their families and carers along with the local community learning disability teams, care providers, inpatient services and social care.

You would join existing nurses in a team where there is strong multi-disciplinary working and where the contribution of nursing is highly valued and you will be working across Coventry and Warwickshire.

Main duties of the job

The team's aim is to empower and promote individuals with a learning disability and complex needs to live meaningful lives. To adopt Positive Behaviour support approaches to reduce length of time in hospital and to support the prevention of inappropriate admissions or readmissions. This will be working within the home environment or appropriate care settings, alongside the wider Multi-disciplinary team.

To support people with a learning disability and/or autism who display behaviour that challenges, including those with a mental health condition.

Assess individuals with significant complex needs and / or challenging behaviours requiring individualised packages of care.

The post holder will be involved in mentoring staff and students.

Essential
  • Nursing Degree in Learning Disability or Mental Health Nursing
  • Registered with the NMC

Desirable
  • Evidence of ongoing CPD
Experience

  • Experience working with people with learning disabilities. Community experience preferred but not essential
