This is an exciting opportunity for individuals interested in palliative and end-of-life care to join our dynamic specialist team.
We are seeking an enthusiastic and highly motivated Senior Healthcare Assistant to join the multi-disciplinary team providing specialist palliative care to patients and their families.
The role is based within the Specialist Palliative Community Hubs, working alongside Clinical Specialist Nurses and Allied Health Professionals to deliver community-based palliative care.
The post requires an experienced Health Care Assistant with a good understanding of end-of-life and palliative care.
The responsibilities include managing a clinical Stable Phased (OACC) Caseload, acting as a key worker for individual patients, and coordinating their specialist palliative care needs, including making referrals to other team members as appropriate. The post involves close collaboration with Clinical Nurse Specialists and Clinical Hub Leads.
Responsibilities also include holistic assessment, supportive mapping, advance care planning, and reviewing stable patients.
The successful candidate must be able to work autonomously, conducting holistic assessments and planning care to meet patients' practical, psychological, spiritual, and social needs. Excellent communication and team-working skills are essential, as is the ability to work closely with primary and secondary care professionals and Hospice colleagues.
