Community Nurse - Band 5Location: Plymouth (Multiple bases including South, East, West, and North DN Teams)

We're looking for a registered Band 5 Community Nurse to join our Adult Frailty Specialist Services team in Plymouth.

You'll provide high-quality, evidence-based care to patients in their homes, working across a 7-day service. The role includes wound care, diabetes management, medication administration, end-of-life care, and more.

What you need:

NMC Registration

Experience in a healthcare setting

Strong clinical and communication skills

Full driving licence and access to a vehicle
