Senior Community Healthcare Assistant
Band 3
34 hours per week
This is an exciting opportunity for individuals interested in palliative and end-of-life care to join our dynamic specialist team.
We are seeking an enthusiastic and highly motivated Senior Healthcare Assistant to join the multi-disciplinary team providing specialist palliative care to patients and their families.
The role is based within the Specialist Palliative Community Hubs, working alongside the Clinical Specialist team of Nurses and AHPs to deliver community-based palliative care.
The post requires an experienced Healthcare Assistant with a good understanding of end-of-life and palliative care.
Responsibilities include managing a clinical Stable Phased (OACC) Caseload, acting as a key worker for individual patients, identifying and coordinating their palliative care needs, and referring to other team members when appropriate, in collaboration with the Clinical Nurse Specialist and Clinical Hub Leads.
The post holder will be responsible for holistic assessments, supportive mapping, advance care planning, and reviewing stable patients.
Working autonomously, the post requires planning care to meet patients' practical, psychological, spiritual, and social needs. Collaboration with primary and secondary care professionals and hospice colleagues is essential, along with good communication and team working skills.
