Job summary

County Durham & Darlington NHS Foundation Trust invites applications for a Clinical Practitioner on a 12-month secondment to join the Families First Partnership Front Door. You will provide senior clinical input, analyse health information, and influence multiagency decision-making for children, young people and families.

You will help design and test the health contribution within a developing multiagency model, work across organisations, and support go-live of the Front Door reforms while

Qualifications

• Two experienced registered health professionals with strong safeguarding knowledge.
• Ability to work autonomously with complex information and across boundaries.
• Commitment to early intervention and child welfare principles.

Responsibilities

• Provide senior clinical input to support decision making for children, young people and families.
• Draw together information from multiple health providers and interpret its significance.
• Develop health contributions to multiagency Front Door and promote timely health actions.
• Collaborate across agencies to strengthen health components of the safeguarding pathway.
