Cheshire East Council is seeking an ambitious Practitioner Support Officer – Integrated Front Door to work from Delamere House, Crewe. The role focuses on Early Help Assessment (EHA) and Lead Professional (LP) processes, providing advice, information and guidance to practitioners and families to improve outcomes for children and young people.

Qualifications

• Experience of advising practitioners to deliver quality services.
• Experience of working in multi-agency partnerships.
• Experience using holistic assessments to plan support for children and families.

Responsibilities

• Provide advice, guidance and practical support to practitioners and managers across agencies.
• Deliver training and briefings on EHA and Lead Professional processes.
• Triaget and assess family needs during home visits.
