A prominent healthcare provider in Portsmouth is seeking an experienced clinical pharmacist to work in a varied, patient-facing role. The successful candidate will provide primary support for prescription queries and help run chronic disease management clinics. Qualifications include a postgraduate diploma in clinical pharmacy and full registration with the General Pharmaceutical Council. The position also offers the chance to engage in quality improvement projects within a supportive multi-disciplinary team environment. To work as part of a multi-disciplinary team in a patient-facing role. Providing primary support to general practice staff with regards to prescription and medication queries. Support the established repeat prescription system (centralised prescription hub), medicines reconciliation on transfer of care and systems for safer prescribing. Run chronic disease management clinics. Provide structured medication reviews, quality improvement projects as well as undertaking some aspects of the quality and outcomes framework (QOF) and enhanced services.
Portsdown Group Practice covers the whole of Portsmouth frompostcodes PO1 to PO7. We operate from 6surgeries: Cosham Park House (PO6 3BG), Crookhorn Lane (PO7 5XP, Heyward Road(PO4 0DY), Kingston Crescent (PO2 8AL), Paulsgrove (PO6 4HJ), and Somerstown(PO5 4EZ).
We are our own Primary Care Network (PCN). We have ClinicalPartners, Salaried GPs and a comprehensive Nursing and extended clinical teamoverseen by our Clinical Team Manager.The team consists of Nurse / Paramedic Practitioners, Registered Nurses,HCAs, first contact Physiotherapists, Podiatrist, Social Prescribersand Care Co-ordinators. The Pharmacy team consists of Clinical Pharmacists and Pharmacy Technicians.
The Practice is well established, financially secure, andsuccessful.
Ensure delivery of safe, evidence based, cost effective prescribing within an overall quality and clinical effectiveness framework.
Reconcile medicines following discharge form hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with community pharmacists and secondary care to ensure patients are supported and receive the medicines they need post discharge
