Job summary

Active Care Group is seeking a Senior Case Manager in London to lead and support a team delivering specialist case management for individuals with complex health conditions. You will manage your own caseload, develop comprehensive plans, and engage with referrers to drive service growth.

You will provide clinical cover, ensure regulatory compliance, and mentor junior staff within a supportive leadership framework.

Qualifications

• Experience delivering specialist case management for complex health conditions.
• Ability to develop case management plans and costed recommendations.
• Strong relationship-building with referrers and partners.

Responsibilities

• Manage own caseload of clients with complex disabilities.
• Develop and maintain Case Management plans with costed recommendations.
• Identify and commission appropriate rehabilitation services.
• Ensure all client documentation is accurate, up to date and compliant with regulations.
• Develop and maintain relationships with stakeholders and referrers.
• Provide ongoing clinical and professional support to case managers and lead when required.
• Maintain training compliance and adherence to group policies.
