Active Care Group's Brownbill Case Management Services is seeking an experienced Case Manager (Lead) to work with the Clinical Lead, guiding Case Managers and developing relationships with referrers. Hybrid role with head office in Thame and case manager coverage across London, Home Counties, Essex, Kent, Midlands and South.

You will manage your own caseload of clients with complex disabilities, ensure documentation compliance, and support clinical governance and service delivery.

Responsibilities

• Manage your own caseload of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families.
• Develop and maintain Case Management plans with costed recommendations.
• Identify and commission rehabilitation services for clients, ensuring effective service delivery and addressing issues.
• Ensure all client documentation is accurate, up-to-date, and compliant with relevant regulations and standards.
• Build and maintain relationships with stakeholders, referrers, and regulatory authorities; attend networking events as required.
• Monitor quality of case management provision and provide clinical support to case managers with regular reviews.
