Job summary

Brownbill Case Management, part of Active Care Group, is seeking an experienced supervisor to guide Case Managers, support clinical practice, and maintain service quality across London, Home Counties, and surrounding regions.

You will manage a caseload, develop care plans, and build relationships with referrers while supporting the Clinical Lead and Senior Leadership Team in delivering high‑quality case management services in clients' homes and community settings.

Qualifications

• Professional healthcare qualification with active registration.
• Strong organisational and multitasking abilities.
• Excellent oral and written communication.

• Ability to build relationships with stakeholders.

• Knowledge of litigation process and safe recruitment.
• CQC regulations knowledge desirable.

Responsibilities

• Manage your own caseload of clients with complex disabilities and set goals with clients and families.
• Develop and maintain case management plans with costed recommendations.
• Identify and commission rehabilitation services and manage issues.
• Ensure all client documentation is accurate and compliant with regulations.
• Develop and maintain relationships with stakeholders, partners and referrers.
• Monitor service quality and provide ongoing clinical and professional support.
• Lead with a supportive management approach to foster innovation and compliance.
• Provide clinical cover for the Clinical Lead when unavailable.
• Ensure colleagues know mechanisms for speaking up in the organisation.
• Maintain training compliance and regulatory standards.
• Adhere to group policies and ensure staff awareness.
