Job summary

Brownbill Case Management Services (BAL), part of the Active Care Group, is seeking a senior leader to guide Case Managers and maintain service quality across the business. You will develop relationships with referrers and oversee caseloads in a hybrid role serving Thame, London and Home Counties, Essex, Kent, Midlands and the South.

Qualifications

  • A recognised professional qualification in a healthcare field with active registration.
  • Knowledge of CQC regulations is desirable.
  • Understanding of changes in case management expectations among stakeholders.

Responsibilities

  • Manage your own caseload of clients with complex disabilities and set goals with clients and families.
  • Develop and maintain Case Management plans with costed recommendations.
  • Identify and commission rehabilitation services and ensure effective delivery.
  • Maintain accurate client documentation compliant with regulations and standards.
  • Build relationships with stakeholders, referrers and regulators at networks and events.
  • Provide ongoing clinical and professional support to case managers and conduct client reviews.

Job description

Brownbill Case Management has been established since 1992 and exists to deliver an outstanding case management service to children, young people and adults who have enduring and complex health conditions and injuries.

As part of the Active Care Group (ACG) Case Management Division, Brownbill Case Management Services (BAL) provides a specialist case management service primarily for individuals who have been affected by brain injury, spinal cord injury, Cerebral Palsy, complex orthopaedic injuries and amputation. We work across the Midlands, London and Home Counties, and South East regions.

In this role, you will work closely with the Clinical Lead to provide guidance and support to Case Managers, promoting best practice and service consistency. Additionally, you will play a key role in developing and maintaining strong relationships with existing and prospective referrers, contributing to the ongoing growth and success of the business. You will be responsible for managing your own caseload while providing support to the Clinical Lead and wider Senior Leadership Team to ensure the delivery of high-quality Case Management services across the business.

This role is hybrid working, head office Thame and case manager coverage London and Home Counties, Essex, Kent, Midlands and South.

Key responsibilities:
  • Manage your own case load of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families.
  • Develop and maintain Case Management plans with costed recommendations.
  • Identify and commission appropriate rehabilitation services for your clients ensuring service delivery is effective and managing any issues that arise.
  • Ensure all client documentation is accurate, up to date, and fully compliant with relevant regulations and standards.
  • Develop and maintain solid relationships with stakeholders, business partners, regulatory authorities and existing and prospective referrers attending networking events as required.
  • Monitor the quality of case management service provision and provide ongoing clinical and professional support to case managers, including regular client reviews and annual appraisals in discussion and agreement with the Clinical Lead.
  • Lead with a supportive management strategy to foster innovation, manage risk, and adhere to professional regulatory organisations' codes of practice.
  • Provide clinical cover for the Clinical Lead in case of leave or illness.
  • Make sure colleagues are aware of the mechanisms for speaking up in ACG and act where required.
  • Maintain your own training compliance and ensure you are up to date with regulatory standards.
  • Adherence to all Group policies and procedures ensuring that all colleagues in the service are aware of and understand company policies.
Knowledge, Skills and Qualifications:
  • A recognised professional qualification in a healthcare fieldsuch as Nursing, Occupational Therapy, Physiotherapy, or Social Workwith active professional registration maintained throughout employment.
  • Strong organisational skills, including the ability to multitask and organise others.
  • Excellent oral and written communication skills.
  • Capable of working under pressure and adapting to team dynamics.
  • Strong attention to detail and deadlines.
  • Skilled at filtering and prioritising information.
  • Capable of building and maintaining positive relationships at all levels, including during difficult or challenging circumstances.
  • Skilled in prioritising and managing workload in high-demand environments.
  • Ability to think ahead and anticipate needs before they arise.
  • Ability to exercise discretion when dealing with confidential or sensitive matters.
  • Confidence and ability to work on own initiative.
  • Knowledge of the litigation process.
  • Knowledge of safe recruitment practices.
  • Knowledge of CQC regulations is desirable.
  • Understanding of changes in case management arena and expectations of case management provision by different stakeholders.
Experience:
  • Proven experience in providing guidance and supervision to a team.
  • A minimum of 2 years' experience within the case management sector in addition to at least 5 years' experience in a relevant professional field.

Brownbill work closely with the client and their family or representative to provide tailored support packages for clients in their own homes and in the community. Brownbill has continued to expand to meet the increasing requirement for case management services and now boasts a multi-disciplined team of 10 case managers plus support staff at head office and manages over 250 care/support workers on behalf of Clients. Brownbill is determined to continually develop in order to provide intervention for as many people as possible.
