Job summary

Brownbill Case Management Services, part of Active Care Group, delivers specialist case management for complex health conditions across the Midlands, London, Home Counties and the South. You will work with the Clinical Lead, manage your own caseload and support senior leaders to ensure high‑quality services in clients0homes and communities.

This hybrid role is based at Thame head office with coverage across London, Home Counties, Essex, Kent, Midlands and South.

Qualifications

• Professional healthcare qualification with active registration.
• Strong organisational and communication skills.
• Ability to work under pressure and meet deadlines.

Responsibilities

• Manage your own caseload of clients with complex disabilities and set goals with clients and families.
• Develop and maintain Case Management plans with costed recommendations.
• Identify and commission appropriate rehabilitation services for clients and ensure effective delivery.
• Ensure all client documentation is accurate, up-to-date and compliant with regulations.
• Develop and maintain relationships with stakeholders, referrers and regulatory authorities.
• Provide ongoing clinical and professional support to case managers and conduct regular client reviews and appraisals.
