Job summary

Prestige Nursing & Care East Lancashire is seeking a Senior Care Coordinator to join the Blackburn office, coordinating staff rotas, referrals and home-based care with a focus on person-centred support.

You will balance office duties with visits to clients’ homes, ensuring safe care, accurate records and timely package mobilisation, while building trusted relationships with families and professionals.

Qualifications

• Experience in care coordination, rostering or a senior care role.
• Excellent organisation, administration and communication skills.
• Calm, practical and solution-focused under pressure.
• Knowledge of person-centred care, safeguarding, risk management and CQC expectations.
• Confident in building professional relationships and handling sensitive information.
• Full UK driving licence and access to own vehicle.

Responsibilities

• Create and maintain safe, effective staff rotas using IRIS Digital Care.
• Coordinate new referrals from initial information through assessment, mobilisation and start of care.
• Carry out care assessments, reviews and manual-handling reviews in clients' homes.
• Keep training, compliance and care documentation accurate, current and review-ready.
• Support recruitment, onboarding and day-to-day engagement of care colleagues.
• Be a responsive point of contact for clients, families, commissioners and professionals.
• Spot risks or service pressures early and escalate appropriately.
• Participate in the shared out-of-hours on-call rota with the branch team.
