Job summary

SELECT LIFESTYLES is seeking a dedicated Care Coordinator to oversee and organise care services across its senior care settings. The role involves coordinating personalised care plans, supervising staff, and ensuring high standards of resident well-being.

You will travel between facilities using a valid driving licence, liaising with residents and families, organising rotas, monitoring health and safety, and using IT systems to record data and generate reports.

Qualifications

• Supervising staff in a care home or senior care setting.
• Strong leadership and organisational skills.
• Excellent communication with residents, families, and colleagues.
• Valid driving licence for travel between sites.
• Experience developing and managing care plans compliant with regulations.
• Proficient with IT systems for recording resident data and generating reports.
• Experience in elder care services is desirable.

Responsibilities

• Develop, implement and review personalised care plans.
• Coordinate daily care activities to ensure seamless service delivery.
• Supervise and support care staff, fostering a team environment with strong leadership.
• Act as primary contact for residents' families, providing updates.
