Job summary

East London NHS Foundation Trust seeks post holders to deliver high quality assessment and treatment interventions to young people as part of the HTT offer. • Deliver mental health assessments for children and young people.
• Provide care plans and risk management across settings (home, community, hospital).
• Collaborate with other professionals and train staff to improve biopsychosocial care.

Responsibilities

• Triage referrals and provide initial advice to referrers.
• Conduct comprehensive mental health and risk assessments.
• Develop and implement risk management and safety planning.
• Deliver brief crisis interventions and facilitate early discharge when possible.
• Engage in assertive outreach and inreach activities as needed.
