Job summary

East London NHS Foundation Trust operates the CRHTT and Kelvin Grove Crisis House, providing 24/7 crisis support for adults. The post holder will assess mental state, risk and care needs, coordinating with GPs, wards and community services for safe discharge where possible.

You will work within a multidisciplinary team, conduct comprehensive risk assessments and deliver psycho-social interventions to promote recovery and reduce re-admission.

Responsibilities

• Assess from various sources including GPs, CRHTT and acute wards to provide home treatment.
• Be part of the assessment or the home treatment team within the Crisis Team.
• Provide gatekeeping and feedback based on CRHTT criteria.
• Provide comprehensive risk assessments incorporating baseline physical assessments.
• Provide expert advice on management, treatment interventions and diagnoses during a mental health crisis.
• Support acute hospital wards and facilitate early discharge when appropriate.
• Use psycho-social interventions to improve mental health outcomes for patients.
• Reduce readmissions by client education and recovery support.
• Provide seamless care from GP or ward to community for service users.
• Act as shift co-ordinator and assign colleagues' work for the day.
• Be aware of and adhere to lone worker policy.
• When required supervise and facilitate the administration of medication.
• Participate in MDT meetings and team/business meetings.
• Promote social inclusion and community participation.
• Act as advocate for patients and carers where appropriate.
