Job summary

Holmlands Medical Centre is seeking an experienced Advanced Practitioner to join our friendly practice. The role covers 3-4 sessions per week with clinical examinations, acute care, and prescribing certification.

You will manage patient care across telephone, face-to-face, and video consultations. We value teamwork, development, and excellent patient care, with a focus on forward planning and initiative within a GP-led centre.

Qualifications

• Advanced clinical practiceskills; management of patients with long-term conditions.
• Clinical leadership and governance responsibilities in nurse-led service.
• Experience in nurse prescribing and minor illness management.

Responsibilities

• Deliver high-quality clinical assessment, diagnosis, and treatment with safe referral.
• Provide same-day assessment appointments and pre-booked chronic disease clinics as applicable.
• Support Duty Team by handling on-day patient demand via telephone, face-to-face, and online consultations.
• Work autonomously while contributing to the wider clinical team.
