Job summary

The Hollies Medical Practice is seeking an Advanced Clinical Practitioner to join our busy team. This full-time role offers 37.5 hours per week, Monday to Friday, working closely with GP partners, pharmacists, nurses and support staff to deliver high-quality care.

The successful candidate will be an experienced prescriber with a Masters-level qualification and will provide clinical leadership, triage patients, and participate in home visits within the catchment area.

Qualifications

• Minimum 2 years general practice experience.
• Experience in managing common minor illnesses.
• Broad knowledge of general practice and chronic conditions.
• Experience with clinical governance and audits.
• Knowledge of evidence-based healthcare.

Responsibilities

• Provide high-quality primary care in a safe practice setting.
• Triage acutely ill patients and manage chronic conditions.
• Lead in prescribing within professional boundaries.
• Collaborate with GPs and multidisciplinary team across the practice.
• Attend home visits and travel within the catchment area.
• Support in quality improvement and CPD activities.
