UHNM in Hanley, Stoke-on-Trent is looking for an enthusiastic Advanced Clinical Practitioner to deliver high-quality, patient-centered care within the Community Rapid Intervention Service and Virtual Ward. The post supports rapid home-based acute care and admission avoidance in collaboration with general acute medicine.

The ACP will work autonomously at an advanced level, with expertise in clinical assessment, prescribing (as an independent prescriber), and coordinating a multidisciplinary team

Qualifikationen

• MSc in Advanced Clinical Practice required.
• Registered Independent Non-Medical Prescriber.
• Excellent clinical knowledge in emergency/acute/general medicine.

Aufgaben

• Provide advanced clinical assessment and management in community/home setting.
• Autonomously formulate management plans and prescribe as applicable.
• Lead and support multidisciplinary teams within the Virtual Ward/CRIS service.
