Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking an Advanced Clinical Practitioner to support a core team of ACPs across the PCN North Hardwick and Bolsover with home visits for seven practices.

You will provide clinical leadership, direct care, and guidance on complex cases, while driving audits, education and quality improvements across adults and children in community settings. This role offers professional autonomy within a collaborative, patient-centered model.

• Advanced clinical expertise and judgement in adult and pediatric care.
• Ability to lead a multi-disciplinary ACP workforce and participate in appraisals.
• Experience with audit, supervision, and quality improvement in community settings.

Responsibilities

• Provide clinical leadership and direct care for adults and children in home visit settings.
• Support ACP workforce development and training needs.
• Lead on audits, performance management, and feedback for staff appraisals.
• Deputise for Clinical Lead when required.

Job description

The role will support a core team of ACPs across our PCN North Hardwick and Bolsover providing home visits for 7 practices As an ACP you will:

• Provide clinical leadership, including direct clinical care, and advice on complex issues
• Establish a cohesive team approach for the ACP workforce development
• Support the home visiting service leads and managers with audit, innovation, and performance management
• Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps
• Assist in feedback for staff appraisals.
• Deputise for the CL where necessary, if not being carried out by other members of staff.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to adults and children, presenting with undifferentiated diagnosis You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults and children. You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.
