Job summary

Central London Community Healthcare is seeking an Advanced Clinical Practitioner to join our Unplanned Care Services across South and West Hertfordshire on a 12-month fixed-term basis. The ACP will lead rapid response and Early Intervention Vehicle activities, delivering advanced clinical assessments and decisions to support home-based, timely discharges.

You will collaborate with a multidisciplinary team and other clinicians, covering weekends and bank holidays as part of a rota, while

Qualifications

• Senior clinician with ACP qualifications and experience.
• Experience in urgent/unplanned care settings preferred.
• Willingness to travel across service area and work weekends/bank holidays.

Responsibilities

• Provide advanced clinical assessment and decision-making within Rapid Response and Early Intervention Vehicle services.
• Support patients to receive safe, effective care at home and enable timely discharge from acute settings.
• Work as part of a multidisciplinary team to strengthen partnerships with primary care and care homes.
