University Hospitals of North Midlands NHS Trust is seeking an Advanced Clinical Practitioner (Band 8a) to join the Acute Care at Home and Community Virtual Ward teams in Stoke-on-Trent. You will provide autonomous clinical assessment, prescribe as an independent Non-Medical Prescriber and lead on complex home-based care for frail patients.

Qualifications
- Registered Healthcare Professional with MSc in Advanced Clinical Practice.
- Independent Non-Medical Prescriber (prescribes as part of role).
- Experience in community/acute care settings is desirable.
Responsabilités
- Provide autonomous, advanced clinical assessment and management in patients' homes.
- Prescribe and review medications within probable scope of practice.
- Lead on development of care plans and coordinate MDT input in CRIS/CVWS.
