Job summary

Shropshire Community Health NHS Trust is recruiting an Advanced Clinical Practitioner (ACP) to join the Urgent and Emergency Care Services in Shrewsbury. The role provides advanced clinical care in a seven-day service, delivering safe alternatives to hospital admission through home visits and digital tools.

You will work within the multidisciplinary team, hold a MSc in Advanced Practice, and be registered with NMC/HCPC.

Qualifications

• MSc qualification in Advanced Practice.
• Registered Nurse or Allied Health Professional with current NMC / HCPC registration.
• Relevant qualifications or demonstrable equivalent experience in speciality (Frailty, Care of the elderly/Geriatric’s, End of Life, ALS, minor injuries/illness)
• Evidence of recent CPD
• Teaching and Mentoring qualification or demonstrable equivalent experience

Responsibilities

• Deliver an advanced level of clinical practice autonomously in general medicine, Frailty, Respiratory, and Cardiology.
• Ensure high quality, person-centred care through advanced clinical judgements, investigations, diagnosis, prescribing, planning, implementing and evaluating care.
• Embrace the four pillars of clinical practice: leadership and management, education and research.
