Job summary

Lancashire County Council's Children and Family Wellbeing Service is recruiting a SEND Support Facilitator in Lancaster on a permanent, full-time basis. You will provide specialist advice, support and consultation to SEND Children, Young People and their families.

You will work with partners to ensure accessible and inclusive local support. You will help develop and deliver inclusive Family Hubs and group work for SEND Children and Young People, liaising with wider SEND services.

Responsibilities

• Provide specialist advice, support and consultation to SEND Children, Young People and families supported by our Family Intensive Support service.
• Offer consultation to team members and partners across the Children and Family Wellbeing Service and Early Help Partnership.
• Signpost SEND Children, Young People & Families who access our Family Hubs and wider service offer.
• Support development and delivery of inclusive Family Hubs and group work provision for SEND Children & Young People.
• Develop links with Lancashire Parent Carer Forum and Family Hub Parent Carer Champions to co-design services.
