Change Grow Live in Brighton is recruiting a Navigator Outreach Worker to engage rough sleepers and connect them with healthcare, housing and social support. You will work on outreach to clients with long, complex housing histories, coordinating with Brighton & Hove City Council and partners.

The role requires excellent communication, initiative and timely follow-through, with co-location across services and a fixed-term contract until March 2029.

Formación

• Experience engaging with rough sleepers or equal vulnerable groups.
• Ability to coordinate with multiple services and agencies.
• Strong communication skills (verbal and written) and IT proficiency.

Responsabilidades

• Engage with clients with complex histories of rough sleeping in Brighton & Hove.
• Identify and contact long-term rough sleepers, providing assertive outreach.
• Ensure access to ongoing services such as healthcare, recovery and social care.
• Collaborate with partner services to implement action plans.
