Change Grow Live in Brighton is looking for a Navigator Outreach Worker to engage rough sleepers and connect them with the right support.

The role is co-located with the Street Outreach Service and the Multi-Disciplinary Enhanced Support Team, focusing on those with long and complex histories of rough sleeping in Brighton & Hove.

This fixed-term contract runs until March 2029, with 37.5 hours per week and a pro-rata approach for part-time posts.

Formación

• Excellent communicatio n skills, both written and verbal.
• Strong time management and prioritisation abilities.
• Good IT skills and comfort with information governance.
• Experience working across multiple agencies and services.
• Ability to work well in a team and build partnerships.
• Proactive, able to work on own initiative and meet deadlines.

Responsabilidades

• Work with an identified number of clients who have repeated, prolonged and/or complex experiences of rough sleeping in Brighton & Hove.
• To identify and establish contact with long-term rough sleepers, providing assertive outreach to individuals who services can have difficulties engaging with.
• Proactively ensure that service users have access to ongoing services such as healthcare, recovery and social care.
• Work closely with all partner services in Brighton and Hove, sharing appropriate information and ensuring that action plans are carried out.
