The Bridge Project is recruiting a full-time Rough Sleeper Recovery Coordinator in Bradford to join the New Vision Bradford partnership. You will act as the main care coordinator, delivering tailored packages of care and supporting service users from entry through to recovery, with outreach to connect with rough sleepers.

You will coordinate assessments, recovery planning, and one-to-one key work, using recognised psychosocial interventions and harm minimisation.

Qualifications

• Excellent communication skills and ability to manage multiple priorities.
• Highly organised with strong time management abilities.
• Ability to work as part of a multi-disciplinary team.

Responsabilités

• Act as the main care coordinator for an assigned group of service users.
• Coordinate tailor-made packages of care and provide vital support.
• Conduct comprehensive assessments and recovery planning.
• Deliver one-to-one key work and recognised psychosocial interventions.
• Harm minimisation and outreach to rough sleepers or those at risk.
