Job summary

The Bridge Project, via the New Vision Bradford partnership, is hiring a Rough Sleeper Recovery Coordinator in Bradford. The role involves acting as the main care coordinator for service users, coordinating tailored care packages and supporting recovery journeys.

Qualifications

Minimum 2 years’ full-time experience with people with alcohol and/or drug problems within a 4-year period.
Experience in case management and delivering care coordination in drug and/or alcohol or related field.
Strong organisational and communication skills.

Responsibilities

Act as the main care coordinator for an assigned group of service users.
Coordinate tailor-made packages of care and provide vital support including recovery planning.
Provide comprehensive assessments and one-to-one key work.
Deliver recognised psychosocial interventions and harm minimisation.
Lead outreach to connect rough sleepers or those at risk with support services.
