Job summary

The Bridge Project in Bradford is recruiting a full-time Rough Sleeper Recovery Coordinator. You will act as the main care coordinator for an assigned group of service users, coordinating tailored care packages and recovery planning, supported by a multidisciplinary team.

The role requires 2 years of relevant experience and a driving licence, with outreach to rough sleepers and those at risk, plus strong organisational and communication skills.

Qualifications

Requires 2 years' FT experience with alcohol/drug problems or complex needs within a 4-year period.
Able to travel across the district with a valid driving licence.
Experience in case management and delivering care coordination in drug/alcohol or related field.
Excellent organisational and communication skills; team-oriented.

Responsibilities

Act as main care coordinator for an assigned group of service users.
Coordinate tailor-made packages of care and recovery planning.
Conduct comprehensive assessments and one-to-one key work.
Deliver recognised psychosocial interventions and harm minimisation.
Conduct outreach to connect with rough sleepers or at-risk individuals.
