Job summary

Forward Trust is recruiting a Rough Sleeper Recovery Worker to support adults 18+ experiencing homelessness in Canterbury. You will work within the Rough Sleepers Initiative to help clients access accommodation, treatment and wider health and housing services.

You will carry a small caseload, deliver person-centred interventions using harm reduction and recovery approaches, and collaborate with outreach workers to build pathways into services.

Qualifications

Experience in substance misuse services and homelessness support.
Ability to engage individuals with complex needs and trauma.
Understanding of harm reduction and recovery approaches.
Ability to assess risks and develop tailored support plans.

Responsibilities

Completing comprehensive assessments and risk management plans.
Delivering harm reduction guidance and one-to-one interventions.
Facilitating group-work and engaging clients in services.
Collaborating with outreach workers and partner agencies.
Coordinating with healthcare, Probation and housing teams.
Ensuring joined-up support for substance misuse, health and housing.
Contributing to targets and measuring outcomes for the initiative.
